STUDENT DROP-OFF CONSENT FORM
Students younger than 12 must be met at the bus when by a parent or guardian when being dropped off. This ensures our youngest students arrive safely at home every day. School bus drivers will not drop young students off alone unless they are older than 12, or they have a family safety plan in place. If you have a created a safety plan for your family, please indicate your preference for school bus drop-off's below. Completion of this form will ensure your child is dropped off safely by the school bus driver. If your child is under the age of 12, please indicate the most appropriate option below. |
SAFETY PLAN IN PLACE
Yes, I have a safety plan in place for my child which allows them to be dropped off at their designated bust stop on their own without a parent, guardian, older sibling, babysitter or neighbour being physically present.
DO NOT DROP-OFF ALONE
No, please keep my child on the bus if I or my designated guardian (as indicated on my child's emergency contact information with the school) are not physically present at the bus stop. I acknowledge that it is my responsibility to pick up my child at their school if the Transportation Department has been unable to reach me at the numbers I have provided.
